Please have your business’s Federal Tax ID number available when completing the form.

You may begin submitting samples while your request is being processed. For guidance on sample submission requirements and available diagnostic tests, please refer to our Laboratory Users Guide and our Search for Diagnostic Test tool for assistance.

Already have an ILVDL account? If you need access to the Client Portal to view test results, please refer to our Client Portal Guide.

If you are with the U of I Veterinary Teaching Hospital and are looking for test results, do not use this form - please call 217-333-1620 or email vdloffice@vetmed.illinois.edu. Please have the accession reference number or PID for your patient available.

A $100.00 fee applies for after-hours drop-off services requiring an on-site Pathologist.
